Advanced Primary Care

Advanced Primary Care (APC) is a Patient-Centered Medical Home and an Accountable Care Organization (ACO) which is Physician owned and led. There is strong evidence showing that supporting primary care where first contact care is comprehensive, continuous, and coordinated will reduce healthcare costs.

The goal of such team based coordinated care is to ensure a blended funding model which combines AHS government funding for INSURED services plus a direct patient care contract for UNINSURED medical services.

​When an ACO succeeds both in delivering high-quality care and spending health care dollars more wisely, the ACO will share in the savings it achieves for the medicare program.
